Updated version of Prostart 4

I have modernised the Prostart prototyping tool with the following.

1. Added support for the msvcrt library and include.
2. Alternate message loops depending on if key processing is required.
3. Rewrote TopXY so it is smaller and without a stack frame.
4. Minor formatting changes.
5. Removed the old debugging macros and DLLs.

It now comes at a disk blowing 28k.  :bg
